To book an appointment, please call us between 8am - 5pm Monday through Thursday. If you would like to leave a voicemail or send an email, we will return your correspondence as soon as we can. Please note that your requested appointment made by email or voicemail is not confirmed until we call you back to confirm, if applicable, the requested date and time. What to Bring to Your Appointment
For your convenience, you may download, print, and fill out the new patient form to bring to your appointment. We ask that you bring your insurance information, valid method of identification, current list of medications, and method of payment to your appointment.
Financing Options
Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. For the convenience of our patients, we accept all major credit cards and offer financing through CareCredit to help make your treatment more affordable! We also offer Senior Citizen discounts! Please ask a member of our staff and we will review our payment options to accommodate your financial needs.
